Starting with a new healthcare practice can feel like a lot. Knowing what usually happens can make the day easier and help you get more out of your time with your provider.
Before you arrive. At Excellent Ultimate Medical Center, you can choose a time and complete a private online intake ahead of your visit. Having your health history ready saves time once you are in the office. We are a self-pay practice and do not bill insurance, and pricing is shared before your visit.
What to bring. A photo ID, a list of any medicines and supplements you take (including doses), known allergies, and any recent records or test results you have. If you are coming for a physical or DOT exam, bring any forms you were given.
During the visit. You will meet with your provider, talk through why you came in, and have time to ask questions. It helps to write your questions down beforehand so nothing is forgotten.
After the visit. Next steps are shared with you after your visit, whether that is a follow-up, a screening, or a plan to check in again.
